Type 2 diabetes, osteoarthritis and stroke recovery provide effective examples of the patient benefits of multidisciplinary care.  The following sections provides information on which allied health professions could be included as part of the team for each condition, the services each allied health professional may provide, trigger points for a potential GP referral to an AHP and trigger points for an AHP to potentially send the patient back to a GP for review.
